Does my pressing plant have to do anything?

Only if you want it to. There is an export made for a pressing plant: one card without a code, plus the code list, so they can print the codes themselves.

How long do the codes work?

As long as you keep the batch. There is no hosting period that runs out.

Can one code be used twice?

Each code works once. The same fan can re-download for 72 hours, up to 5 times, so a dropped connection does not burn a card.

What about CD and cassette?

The same thing. The card does not care what it travels with.

How the code gets into the sleeve

However it suits your release. The card is 85 × 55 mm, the size everyone knows from a business card.

As an insert in the sleeve, printed ten to an A4 sheet.
As a sticker on the shrink wrap, printed on label sheets.
Printed by a print shop, one card per page with bleed.
Or by your pressing plant: they get a template and the code list.

Why a code and not a link

A printed link is the same for everyone. A photo of it on social media, and the record you pressed is a free download.

Each code works once, then for 72 hours and at most 5 times for the same fan.
You see how many codes have been redeemed.
A code that ends up somewhere it should not can be blocked.
The file stays online as long as your codes exist.
